Home Care
Home Care provides client-centered short- and long-term care, for eligible individuals of all ages and their families to maximize health and/or social well-being, and to allow them to remain in their own homes and the community.
Services include:
- case management
- home nursing
- community mental health nursing
- personal care and assistance with activities of daily living
- in-home respite care
- nutrition
- palliative care
- rehabilitation
- social work
- teaching and family support
- meals on wheels
- equipment loan coordination and foot care.
Referrals are accepted from any source; however, physician referrals are required for palliative care, foot care and the home intravenous programs.
