Population Health
The Community Health Services Division is comprised of the following service areas:
Home and Community Care
Home and Community Care provides client-centered short- and long-term care, for eligible individuals of all ages and their families to maximize health and/or social well-being, and to allow them to remain in their own homes and the community.
Services include:
- case management
- home nursing
- community mental health nursing
- personal care and assistance with activities of daily living
- in-home respite services
- nutrition
- palliative care
- rehabilitation
- social work
- teaching and family support
- meals on wheels
- equipment loan coordination and foot care.
Referrals are accepted from any source; however, physician referrals are required for palliative care, foot care and the home intravenous programs.

Public Health
The Public Health Unit provides public health nursing programs and services, which promote and preserve health, prevent and reduce the incidence of communicable diseases, illness, disability and death across all age groups.
This unit offers a variety of programs:
- Communicable Disease: adult immunizations, communicable disease follow-up and monitoring, tuberculosis (TB), travel health clinics, sexually transmitted infections and community outreach activities.
- Maternal/Child: pre-natal and post-natal classes, home visiting of families with new babies, breastfeeding support and well child clinics which include growth and development assessments and immunizations.
- School Health: hearing and vision screening, speech assessment, immunizations, health information resources, and health education classes.
- Healthy Family Program: A voluntary home visitation program for families with young children.
The Public Health program is used as a teaching facility for the Aurora College Nursing Program and for nurses who are completing post-RN programs.
The Yellowknife Health and Social Services Public Health Unit is open from Monday to Friday 8:30 am to 4:30 pm. It is closed from 12:00 pm to 1:00 pm on Monday, Wednesday and Thursday.

Integrated Services
Integrated Services is located on the second floor of the Jan Stirling building at 4702 Franklin Ave.
Programs in the Integrated Services unit include:
- Community Dietitian
- Canada Prenatal Nutrition Program
- Diabetes Program
- French Languages Servics
- Health Promotion
- Language and Culture
We work with community groups and agencies, schools, GNWT departments, aboriginal organizations and other YHSSA divisions.
We are mandated to address the health and wellness needs of the families and communities residing within the YHSSA service jurisdiction.
In partnership with other groups, we develop, implement and evaluate population health initiatives with a focus on communicable and chronic disease prevention and management, prenatal and infant health, injury prevention, and language and culture.
