Licensing - Registration Requirements
To practise medicine in the NWT, physicians must be licensed by the Medical Registration Committee of the Northwest Territories. The Committee requires the information listed below in order to consider your application.An application package, including all necessary forms, is available from the Registrar, Professional Licensing. The completed package must include:

Completed application with photograph attached (incomplete applications will not be processed).

Specified location of work (please specify name of the clinic or hospital) and planned start date.

Certified copy of medical degree (certified by a Notary Public or Commissioner for Oaths); or a transcript of records which should be mailed directly to the Registrar from the Medical School. NOTE: If Medical training was not in English, proof of fluency in speaking and understanding the English language will be required (submit results of Test of English as a Foreign Language (TOEFL).

Photocopy of Licentiate of the Medical Council of Canada (LMCC).

a) Photocopy of College of Family Physicians of Canada certificate; or proof of completion of two years post-graduate internship in Family Medicine, with rotations in medicine, surgery, paediatrics, obstetrics, gynaecology and psychiatry. NOTE: Physicians who are fully licensed in any Canadian jurisdiction as of November 1999 with one year of post-graduate training will be considered as having completed the two year residency requirement as outlined above. b) If the applicant is applying as a specialist, proof of successful completion of post-graduate specialty program approved by the Royal College of Physicians and Surgeons of Canada or the Professional Corporation of Physicians of Quebec and proof of being licensed in a Canadian jurisdiction as a specialist.

Certificates of Standing from all licensing authorities that the applicant is currently and/or was previously licensed, sent directly to Registrar from the licensing authority. NOTE: If applying for hospital/health authority privileges, have a copy sent directly to them. The Registrar cannot provide anyone with copies. NOTE: If not licensed in Canada, applicants must provide proof of eligibility to be licensed in a province. Sending a letter from a provincial College of Physicians and Surgeons can provide proof of eligibility.

Up-to-date and detailed curriculum vitae/resume.

Three references from individuals who can comment on the applicant's work, sent directly to the Registrar. Reference forms are supplied with the application package. Referees must have worked with the applicant within the past three years. NOTE: If applying for hospital/health authority privileges, have separate references sent directly to them. The Registrar cannot provide anyone with copies.

Photocopy of proof of citizenship i.e. Canadian birth certificate, or valid immigration document.

Cheque, Money Order, or Visa Authorization payable to the Government of the NWT, for one of the following:

Limited Permit - $50.00, valid for three months from the date of issue (not renewable, may be issued twice in a fiscal year).

One-Year Permit - $100.00, valid for one year from the date of issue (not renewable, issued one time only).

Registration and Annual License - $300.00, annual license expires March 31 following the date of issue.
